"Delayed Certificate of Birth" of Mildred Frances Self. In 1965, Mildred asked her father, David Edgar Self, to help her obtain her birth certificate. That’s when she found out that her name was really Frances Mildred Self and not Mildred Frances Self. She lived from 1917 to 1965, for 48 years, without knowing this information. The building which stored the Parish’s birth records had a fire and her birth record had been burnt in the fire. Since she already had a social security card in the name of Mildred Frances Self, she decided to go with that name. The "Delayed Certificate of Birth" was obtained in 1965 and seems to have been based on her: (1) high school diploma and (2) Application for Social Security.
